Some issues can take many weeks and meetings. I would like to be able to add existing issues to a meeting as a way of forming an agenda and having the appropriate documents immediately accessible

The integration of different parts of the system is something we look at continually. Already now you can create documents in meetings. Or create the Agenda in Documents and link it in the meeting's document tab. In the document (agenda?) you can link to issues, documents and tasks.
When is the bane of my life. Honestly, neither myself nor anyone else can say if, when or how this may be implemeneted in Projectplace. All I can promise is that you increase the chances of having this implemented by voting (and getting other people to vote) here on Projectplace Ideas.

Yes, you can combine these ideas. We need to link all other resources to meetings. After all a meeting should address issues, documents, tasks that exist in the project already. Having to add new resources creates duplication.
Ideas with status Business Case require a bit more development and might affect several parts of the system. These will therefore have to be prioritised against other new development. The way we do this is by writing a business case for the development and then discussing the prioritisation of the business cases. We write business cases for the top ideas in this category.
